Worker crushed to death

A 20-year-old construction worker was killed on Tuesday at a housing development site in Brampton, west of Toronto. The man was reportedly operating a boom lift at the site when it came into contact with a house. The man sustained traumatic crush injuries in the incident and was declared dead at the scene.

Texas, USA
A contracted maintenance worker was killed early on Wednesday morning at an oil refinery in Beaumont after being struck by a pipe during maintenance activities at the facility. According to local reports, works were being conducted on a heat exchanger on an elevated platform when a section of pipe fell and struck one of the workers.
Worker crushed by equipment
Texas, USA
A 36-year-old plant operator was killed on Wednesday after being crushed by toppling equipment at a residential development site in Rowlett, north east of Dallas. The man was using a telehandler forklift to unload a truck on a level concrete section at the site when it overturned, pinning him underneath. The man was pronounced dead at the scene. Investigators are as yet unsure if the man jumped form the equipment prior to it toppling over, or if he was thrown from the equipment during the incident.
Worker crushed by falling material
Tennessee, USA
A 55-year-old man was killed on Tuesday night at a steel mill in Lonsdale, north of Knoxville, after being struck by falling material. The man was working in a storage area at the facility when a pallet containing bags of sand-like material fell on him. Investigators have estimated the mass of the pallet to be approximately 1,800kg (2 US tons). Although first responders were able to retrieve the man from under the material, he was declared dead at the scene.
Worker killed in fall from height incident
Texas, USA
A construction worker was killed on Wednesday morning at an apartment complex construction site in a north western suburb of Houston after falling from a temporary elevating work platform. The man and a co-worker were inside a wooden crate being lifted by a forklift to the second floor of the building when the box became unstable. One man jumped from the unstable platform to the second floor of the building, avoiding injury. The second man fell to the ground and was then struck and killed by the falling box.
